How to complete your pledge form/How to make your pledge:

**Write your Employee ID Number in the Social Security Number field on the Pledge Form. As of July 2024, the State Comptroller’s Office is phasing out the use of employee social security numbers for payroll deduction purposes.

  1. Choose an agency to which you wish to give. You may search our online database here. You may choose from organizations providing services in the local areas listed or organizations providing services or a state-wide or national basis.
    If your favorite charity is not listed, contact the SCC office.  We may be able to help. For each charitable organization, a brief description of services provided by the agency, its phone number, 6-digit agency code and the percent of overhead is given.
  2. You are encouraged to designate your gift. Undesignated contributions will be distributed among participating agencies in the same proportion they received designated contributions.
  3. Both a printed and online pledge form are available. Complete your pledge form including the TOTAL dollar amount you want to go to each agency/federation.
    If you would like to designate your gift to a specific program or would like to make it in honor of someone, please attach the Earmark/Memorial Contribution attachment.
  4. **Payroll Deduction: write your employee ID number in the corresponding field on the pledge form for payroll deduction. This number can be found in multiple places, primarily your paystub.
  5. Cash or check: simply submit your Coordinator or Keyworker along with your pledge form.
  6. Credit card: an online pledge form with credit card processing is available at State Combined Campaign PayPal Giving Fund.
  7. Print your email address or home address on the pledge form if you want to hear from the agency(ies) to whom you designated your gift ($25 minimum please).
  8. Mail your pledge card either to your Statewide Coordinator or Keyworker.  You can also email, fax (334-269-4410), or mail your pledge form to the SCC office at [email protected] or 624 South Perry Street, Montgomery, AL 36104.
